Responses are scored and converted to a scale from 1 to 5, with 5 being the highest rating.
  1. Did the provider explain things in a way that was easy to understand?
  2. Did the provider listen carefully to you?
  3. Did this provider show respect for what you had to say?
  4. How would you rate the provider on a scale from 0 to 10, with 10 being the best possible rating?
